  • 24th Season of Revive at Five - 2021 Food Vendor Application

    Please answer all required questions with Yes or No. Vendors will be selected based on highest scores. In the event that scores are tied the final decision will be made by the staff of Downtown Helena, Inc. Once accepted you will be required to sign a formal contract detailing expectations. If selected, the total cost for annual vending is $599. An additional $250 deposit will be required. If you do not break your contract, the $250 will be returned to you at the end of the season.

    2021 Revive at Five application fee of $25 is non-refundable
